Baptism Application

Please fill this form out as completely as possible. Please fill out your contact info if you are the Adult getting baptized, or fill it out for your Child if you are the parent or guardian of the child being baptized. There are 3 Godparent "slots" on this application. For adults and children we suggest having at least one Godparent. There is no limit but we suggest 3 or 4.  If you wish more than three, please fill out the top three and then send an email with the additional information to: [email protected]
